Matsunaga is one of the few I've seen who actually work a decent match with the gimmicks and not just carve his opponent with a sickle(i.e. that worthless sack of crap Pogo). Masato Tanaka is also a top garbage performer for that same reason. Sabu certainly has to be considered, for overall innovation if nothing else. And of course Terry Funk for bringing garbage to the United States.
IMO, the Best is Tomoaki Honma, ok he's in AJ now, but he wrestled a lot of great deathmatches with Shadow WX and Ryuji Yamakawa a couple of years ago... he did a lot of really cool moves with barbed wire and lightbulbs(like the tombstone through a board full of lightbulbs off the apron)
Gotta be Onita, he's the king! It just sucks how everyone I know thinks Mick Foley is the *** of DM, yet they've never seen anything from FMW or whatever, silly WWF/E!
Honma sounds cool, he's the only DM wrestler I know who doesn't wear a shirt for matches Hopefully he does well in AJPW
I hope some time next week I get my Big Japan tapes. I'm going to get the chance to see how good Honma, Yamakawa, Shadow WX, and Winger are. I'm also get to see the 1/2/00 Honam Yamakawa Death Match and get to see WX's Fire Inccident.
Over in the states Id have to give it to the Sandman or maybe Sabu

In garbage wrestling, Onita is the name synonamous with Death Matches. Mick Foley won the IWA KOTDM in 1995 but he has only fought Onita one time that I am aware of and quite frankly I don't think even Cactus Jack could handle Onita. Matsunaga would have to be a very close second, I mean the man has been chewed up by Pirahnas (*wink, wink*) but like someone said before he can carry a decent match in those types of environments.
Onita is the best in my book, he is the walking example of a great "garbage" wrestler. As for "hardcore" in the US, I would have to say Sabu, he combines several skills into his matches and is almost always entertaining to watch.
I feel that I do have to mention Mick Foley however, he does not make it into my top spot for a "garbage" wrestler, but I would say is one of the best bump takers that ever wrestled.
Abdullah the Butcher was pretty awesome in the 70's and 80's in Death Matches. He pretty much made every match he was in a garbage match and was **** effective at them. I used to especially like the fact when he would break out his Judo submission tactics while still using the fork on his opponents. As to how tough he was in his prime, you need only look at his match with Terry Funk in Georgia, about 1976. It ended up being a handicap match when brother Dory wouldn't stay put and got carved up as well.
